<p>I think the deadline for submitting this form is July 1. I am not in the US throughout this summer and the earliest possibility of me getting there is August 28 when I fly to D.C. for Georgetown. Even if I was there, I would not be able to fill the Gtown form out and let the health care provider sign it for me. That's because all of my immunizations - which are reflected on the Dept. of State form - are completed either in my homecountry, or in various US states. I cannot go place to place just asking for people's signatures. Can I just submit the state dept form listing all vaccines instead of the Georgetown immunization form? Who do I even contact on these matters?</p>

<p>^well, I am unable to talk to a real person through any of the two numbers. The immunization department has a voice mail but I will have a hard time explaining my situation in a voicemail, I feel that it will be a complete waste of time.</p>

<p>A. The Immunization and Tuberculosis Screening Certificate must be submitted by July 1st for the fall semester, or January 1st for the spring semester (if you are a student starting in January). If for some reason you are unable to get the form to us by those dates, you must submit it as soon as possible. After August 15th the University Registrar will block continuation of your registration for the fall semester until the form has been submitted and immunization and screening requirements have been met.</p>
<p>In other words, don’t show up without dealing with it. If you’re not going to have it in on time, you need to talk to someone. Leave a voicemail if that’s your only option. The whole immunization form isn’t just some Georgetown thing, it’s actually a DC law that colleges must collect and store vaccination information, so they take it fairly seriously. I’m sure, though, that if you explain your situation, someone will be understanding.</p>
<p>The number I get is on Student Health Center website, so I do not know why there is no real staff in the line to answer your question. Do you call on their office hour? Have you tried to call them again?
Another way, which I use when I do not know where I should call, is calling admission office and they will connect you to the office on duty.</p>
