Common App Teacher Evaluation Form

I have a quick question. So I requested two teachers to do my letter of rec and also the evaluation form on common app/naviance. My one teacher submitted the letter and the form, and it shows up on common app as “assigned” on a certain date and then “downloaded” on a certain date. Today, my other teacher submitted his letter but his name is not on common app at all for the teacher form even though he said he did it. Is my counselor supposed to send it? Why did one teacher’s name show up but not the other?
