<p>I don't think my doctor will be able to complete his forms until tomorrow or Friday. I know that today is the deadline to mail it in. What are the consequences of submitting the forms a few days late?</p>

CALL THEM and tell them about it. They will likely tell you to mail the main forms first if you haven’t done so already and will accept your doctor’s forms later.</p>
<p>I called and the lady said just to get them in as soon as I could. Nothing was said about consequences.</p>
<p>The paper mentions a $100 late fee if you miss something and don’t send it by august. I don’t know if you’ll pay the fee if you send everything late…</p>
<p>haha, actually my doctor filled out the forms and I was able to turn everything in today.</p>
<p>A reminder - don’t forget to go online to fill out your exemption for health insurance (you need to show proof of insurance), otherwise there will be an extra $1000+ on your bursar.</p>
<p>That waiver form isn’t even available until July 21. I had just asked my S about it.</p>
<p>I submitted late last yr - as long as it isn’t as late as July 21, you’re fine.</p>
<p>My D’s doctor won’t fill the form out without giving a physical, earliest we could get an appointment is 8/2. We submitted the online form now & will send in the signed hard copy in August.</p>
