I have looked on admissions requirements on some university websites. Most require 4 units of math and english, but they only look at courses from 9th to 11th. So I have taken 3 years of math and English by the end of my junior year, but I am going to take my fourth year of math and English senior year. Did I fall short of the admission requirements by not having 4 years of English and math done by junior year?

No. You’re fine. Schools will see the courses you are enrolled in the fall of your senior year and also when you have graduated.

No, the course requirements are by the time you graduate, not apply.
