I sent my paper application earlier today, and it just dawned on me that I did not do the ‘most important moment in history’ and ‘tell us something more about yourself’ questions. Should I type responses for these questions and send them out on the 3rd? I need to know what I should do…

Could someone give me the EXACT prompts for both of the questions? Along with the number of each question, too.

What is the address for paper applications? I don't know anymore since I sent everything out.

Thank you.

Should I add a short explanation or something? I can't believe I forgot those two questions. I don't think it's too big of a deal, though...Am I right for not worrying about it too much?

put a quick cover letter, business form, to the dir of admis.
questions: tell us something about yourself that is not readily apparent from your application
in your opinion what was the most important moment in history and why?

You can download the paper app there... it has addresses and exact prompts.

good luck! People make errors.. i'm sure they have these sorts of problems by the thousands. but definitely call- it shows responsibility for your mistake.

Here's what I plan to do: Write a cover letter briefly explaining what happened. Attach the first page of the form 1b (I will fill out name, address, etc. at the top) and answer the two questions. After sending it, I will call the admissions office informing them (Might as well do this on Monday since they are off right now).

Would this be satisfactory? I just dont want to **** this up anymore than I already have!
