<p>Are you talking about the Dean's Evaluation form? If so, common app has a form under school forms that you need to print out and have the Dean or advisor fill out (I'd recommend the Dean). However, if your college is not on the common app, this process will be pretty close to the same as common app. Most of the colleges I have dealt with, that do not sure common app, have a Dean's Evaluation form within the application. Do the same with all the forms regardless of where they came from. Remember, one for each college- one common app form doesn't cover all your schools.</p>
<p>The CA has the College Official's form. This form has questions about your standing, coursework, ratings, etc. for your counselor or dean to fill out. The schools with their own application will have a similar form.</p>

<p>to be absolutely sure of that you should email them. for the common app schools, yes, but i'd be weary about sending an incorrect document.</p>
<p>The common app form says "attach an offical transcript to this form". My dean will not be doing that, as I am requesting the transcripts seperately. Is that a problem?</p>
<p>ugh i went to the dean of students office today to try and schedule an appointment with a dean and the lady said they would only be able to say i was in good standing and not answer the questions detailing my reasons and the more about me type stuff which uchicago has like an entire page of questions about..
is this not ridiculous?</p>

<p>I had the same thing happen to me. At my school they apparently just use a form letter and say that I'm in good academic standing and maybe that I'm on the Dean's List or something. The Dean doesn't even write the letter, it's written by the academic support staff and just signed by the dean so he never even reads it. The person I talked to assured me that colleges aren't expecting anything above that, as the main things they're looking for is disciplinary record, academic standing, and whether I'd be eligible to come back to school or not. Hope that eases your worries!</p>
