<p>The CO form is filled out by someone at the school who has access to your school records (grades, disciplinary record, etc.), so this usually your advisor, Dean of your school or something similar. The fact that the Dean doesn’t know you doesn’t matter much, colleges know this and don’t care, this form is more to see if you are in good standing at your current college.</p>
<p>LORs from instructors should use the IE form.</p>

<p>But there is an evaluation part in the form. Since college official won’t write anything for me, is it better to include a letter from someone who knows me? Or should I just leave it blank?</p>
<p>The form says: “Evaluation Please provide comments that will help us differentiate this student from others. Feel free to attach an additional sheet or another reference you’ve prepared for this student. We especially welcome a broad-based assessment and encourage you to consider describing or addressing:”</p>
<p>That description is aimed at the person filling out and signing the form, not another individual. </p>
<p>You are reading way too much into the description. I have already stated that the CO evaluation is not important, colleges just want to know that you are in good standing at your current school.</p>
