I’m very confused on what it is, I wasn’t aware of it (neither was my counsellor) until after I submitted my application and got the missing documents notification from the applicant portal. What exactly is it and how do I submit it? Just ask my counsellor to email the document? When is it due?
According to the website, the “form” is a section of the School Report that is submitted by your counselor. If you are still in contact with your counselor this week, maybe you can nicely ask him/her to take another look at the report. The website does not seem to give a deadline date, but it does say that an admission decision will not be released without it. Also, don’t be afraid to try contacting your Pomona admissions officer (that’s what they’re there for).
