I checked and rechecked the site over and over and I can’t believe I missed that one!
Actually though, I’m not sure If my GC even needs to fill that out… My school sent out my transcripts along with what I’m pretty sure is a packet of my school’s course information, how many APs, honors, etc… Hmmm… </p>

<p>–> I’m pretty sure your GC should stick it in their recommendation letter envelope. The bottom of the form mentions somethings in regards to the student so I’m assuming the college is preferring the GC sending it with their own letters. (ie. their common app teacher evaluation with letter)</p>
<p>kimcat - Since you’re sending from China, apparently, I think it would be fine to enclose the report with your application. Just to maintain confidentiality, though, you could put it in a sealed envelope and have your counselor sign over the seal. Then put that in your packet so Pomona gets everything at once. </p>
<p>DBang - Pomona is serious about receiving this counselor form. When my son applied a couple years ago, I remember that many applicants posted here about having forgotten to submit the form. Pomona asked them for it, and allowed late submission. I’m sure the college would be even happier if you submit it just a few days late, and they didn’t have to remind you about it!</p>

Also for Dbang: you can find that additional form on the when you click Pomona College on common app. Below the ‘pomona supplement’,there are two separate forms.</p>
