Do I have to have an actual letter of recommendation?

<p>I'm applying using the Common App at a couple of schools. I'm just not sure about one thing:</p>

<p>Do I just need to have the teachers and counselor fill out the evaluation form provided, or do they need to type out an actual letter of recommendation to go with the form?</p>

<p>They just fill out the Teacher Evaluation form. On it, there is a spot where they will write their actual “rec”</p>
