<p>There is usually no form for a non-teacher/non counselor rec. Instead, just have the recommender compose a letter. The best way to do it so that it has the most legitimacy is to have the recommender write it, seal it in an envelope, then send it to your HS guidance counselor. Then have your school send it along with your transcript.</p>

<p>As far as how much influence such a rec may have, the issue is whether this recommender has anything new or different to say than your teachers or guidance counselor. If they do, then definitely get that rec. If this person will only be repeating the same old thing, it's not worth the effort and the college will not really appreciate it.</p>
