<p>I once read on CC and downloaded an extra form for Harvard that is often neglected by applicants. But I lost it so what is it?</p>

<p>wait, do we have to have this form? or is it optional?! Also, do the teacher recommendations on the common app count as the two Harvard wants? Do I have to submit them separately?</p>

<p>To clarify for everyone, yes, that form is mandatory and cannot be neglected. It must be turned in with the SSR (and has to have been by this point if you want to meet the deadline since I am assuming you guys are on break so your counselors won’t be able to do it until after Jan 1).</p>
