Form 5 Questions

<p>Hey all. I realize tons of questions are being posted on here right now. I tried to scan / search but didn't see this really answered anywhere...</p>

<p>I realize I need to fill out Form 5 to self-report my SAT scores and then fax it. My question is, when the form tells me to photocopy my scores, just what do I photocopy exactly? Will any page on College Board that lists my scores suffice? Could I photocopy the page that says what scores I have sent to USC? I chose to not send all of my scores, so I only want to photocopy what was actually sent to them via College Board (I'd prefer there be no evidence of the scores I chose not to send)...</p>

<p>Also, I just need to worry about photocopying SAT / SAT Subject scores that I sent right? I don't need to worry about providing a photocopy of my AP score report? </p>

<p>One last question - as long as my school postmarks forms 4 and 7 by December 1st I am O.K. correct? My application will be submitted tomorrow online, and scores likely faxed tomorrow as well, but my school probably won't send the scores until Monday, November 30th. </p>

<p>Score choice is new this year, so it is hard to answer your question. In years past, you would photocopy your student score report or print the online scores page. If there is an online page that shows the information you would like to present, print it. <p>And postmarked November 30 should be fine.</p>
