<p>does the teacher's form completely serve as a formal letter of recommendation? if i have a teacher fill it out (who is also writing my formal letter for my common app schools), do they also attach a letter? and if so, are additional recommendations required? thanks</p>
<p>That is the only form you need, one for your counselor and one for a teacher. They have space provided on that sheet to write a recommendation. They do not ask for any extra recommendations but, as it is the case with every college, you can send in additional letters if you think you would definitely benefit from it.</p>
