<p>I sent in a supp. rec. and wondering how it should have been labeled. There weren't explicit instructions about this on the website or anything, so I had my recommender state my first and last name and my high school in the first paragraph. Was this sufficient? Thanks!</p>
<p>Well, there's nothing you can do about it now, and they can definitely match it up to your application. Putting your social security # might have helped (if you have one), but I definitely wouldn't worry about it.</p>
<p>Yeah I was wondering about this myself. My admissions counselor recommended I send in a third supplementary recommendation. I want to get one from a non-academic teacher (like my art or drama teacher) but firstly I don't know whether they should get a Common App critique sheet or whether I can send a non-academic recommendation since I already sent in the two required academic recommendations. <p>They can send a regular letter- you're not supposed to use the same sheet as the one they give for the normal academic recs. I'd recommend they write it on stationery with your high school's letterhead, though that's not necessary.</p>
<p>It might be a good idea to ask him to include your social security number, it's easier for them to look up that stuff.</p>
<p>I've specifically phoned Harvard Admissions in Dec. regarding this issue, they said just put your first and last name (in the EXACT way as you spelt it on your application - because I go by a different name spelling at school from my legal name, I actually had to get the letters redone), your high school name, and your social security # (if you have one) in the letter.</p>

<p>I didn't put anything in the letter itself, but I had a separate sheet of paper in the same envelope (either stapled or paper-clipped) that had my full name, address, birthdate, school name, and SSN. I figured that was enough information for them to file it.</p>

<p>oh, i got an email response from their help email line, it says
"If you submit supplementary materials or materials separate from the application forms, please write your full name and secondary school on all supplementary materials and underline your family name."</p>
<p>I didnt include my ssn but it seems like its okay if you dont have it, they didnt mention anything about ssn's in the email</p>
