<p>This is going to seem like a silly question, but will UF take late immunization forms?
Annoying circumstances have lead to me having to choose an earlier preview date than intended, and I will not have the three weeks time to send it in addition to getting the shots.</p>
<p>go get your immunizations and bring the form that uf sends you about immunizations. The form has a fax number on the bottom, when you come back when your done with your three day TB screen and the rest of the shots, have them fax it straight to that number.</p>
<p>That’s what I did, and my immunization hold was gone within hours.</p>
<p>You can actually turn in your immunization forms at Preview. I went to preview last week and people who didn’t have them in before just turned it in right there and everything was fine. Don’t worry too much if won’t have it ready 3 weeks in advance. Just make sure you have all of the required vaccines and signatures and submit it or just bring it to Preview.</p>
<p>What exactly do we have to bring regarding the medical stuff to Preview? I keep reading different things.</p>
<p>Do you absolutely need to get the TB screen? I read that it was only necessary for medical and international students.</p>

Does anyone know if the immunizations that were required as an infant (MMR, Hepatitis B) have to be verified by the doctor who gave them? Or can I just ask the physician I see now to write it in and stamp it?</p>
<p>@strikeb4ck: go to shcc.ufl.edu, and download the mandatory inmunization health history form. Then fax or bring of to your doctor and they have to fill in the necessary info and certify it. Then just make a couple of copies for your records and either fax or mail the form (fax is much quicker) to UF at the number below</p>
<p>@reyes003: you don’t really need it if your major is not on a track for pre-med, but your doctor may still recommend it</p>
