<p>I just went to download the form (Immunizations:</a> Requirements & Form » Student Health Care Center Phone: (352) 392-1161 » College of Medicine » University of Florida) because I am getting my immunizations tomorrow, and I read this part -</p>
<p>"Have a doctors office, clinic or health department fill out the medical areas of the UF Mandatory Immunization Health History Form. Mail OR fax only the single records page (and lab reports as needed) to the UF Student Health Care Center (SHCC) at least 3 weeks prior to orientation/registration"</p>
<p>I have preview exactly two weeks from now. Should I call them after I fax the forms just to make sure it is okay? Am I screwed?</p>

<p>The form makes it appear that the Rubella vaccine is not required, but the website states that “1 Rubella (German Measles) vaccine” is required.</p>
<p>I did not get the Rubella vaccine. I am a male, if this matters at all. </p>
<p>Does anyone know whether it is required or not? It is under the MMR immunization as an “or” alternative to MMR. However, the website says it’s required regardless.</p>
<p>One final question: There is a signature line under the “Required Vaccinations” section. Is the signature mandatory for everyone, or only if you are signing a waiver?</p>

<p>Got everything figured out. The signature is only needed for a waiver, and the MMR’s replace the need for a Rubella vaccine.</p>
<p>This might sound like an obvious question but if we have already gotten the first two required immunizations how do we get a new doctor to sign off on those? Do we bring our old records to any clinic?</p>
