I recently received an evaluation letter from my teacher for the EPGY summer program. I was wondering: would it be appropriate to submit this as sort of a "supplemental recommendation" in lieu of a letter specifically written for a college letter of recomendation?

Sure, provided it is going to tell MIT something that won’t be found in the other two teacher recommendations. Usually, supplemental recommendations are from people who know you in a non-academic context (given 2 academic letters already), say from a sports coach, or a band leader, or an employer, or whatever. However, the guidelines are non-prescriptive.

If you think that this letter will reveal aspects of yourself that the other two teacher recommendations cannot, then you can send it, and MIT will read it. However, if it simply reinforces the other teacher recommendations, MIT may wonder why you did not follow the instructions.
