I don't know which category I should put NHS under.

If it's an honor, is it a regional, state, or national honor?

if it's an activity, what do I say for weeks per year/hours per week if we don't do things regularly and it varies greatly depending on the service project we're working on at the time?

NHS is a school honor. If you do significant work (although it sounds like you don’t) for NHS, feel free to list it under activities. If you have a leadership position in NHS, I would also list it under activities. If you don’t do regular hours or have a leadership position, I would just list it under honors.

What most people do is look at it in the context of their school - at my school, it would be an honour (we get inducted and that’s that - no meetings, etc.). At my friend’s school, they get inducted and do monthly-ish community service projects together, so that would be more of an activity.

Take an average for the numbers of hours. If you do, just for example, 4 hours a month, I’d put 1 hour per week, and then put how many months you do it times 4.
