As Overcore has pointed out meaning is always contextual – and in this case, the context clearly suggests a literal “looking over.” It’s not confusing, because there’s nothing else in the paragraph to suggest that the OP is ignoring anything.</p>
<p>It doesn’t explicitly suggest the ‘look over’ definition. However through process of elimination, we can immediately assume the other two, miss by accident or on purpose, are incorrect. Since he described the patient’s fight vividly, we can only assume that he didn’t miss a thing.</p>
