Kinda worried for my future?

I just finished 11th grade and I didn't do as well as I had wanted to. I took 2 hard classes this year and they were my downfall. I was all 3 years of high school, and took 1 AP class this year. I had a 3.5 gpa freshman year, a 3.4 gpa sophomore year, and a 2.8 junior year.

Junior year I took:
honors calculus (D)
honors physics (D)
AP us history
honors English
honors Chinese
anatomy and physiology

All my other classes were good but calculus and physics. Next year, I'm taking AP Language, AP Lit, AP Chinese, AP Bio, and AP environmental science.

For extracurriculurs - I've been in the marching band for 3 years, I'm the founder of a club that encourages education in 3rd world countries, I'm in the National Honors Language Society, I'm the editor of art for my school's newspaper, I'm an officer in one of my school's cultural clubs, I've been competing in piano for 9 years and I have certificates, and I've also been volunteering at a hospital for 3 years (200+ hours) on the nursing unit, at the information desk, and in the ER. I also hold a part-time job as a tutor at Kumon.

I got a 1930 on the SAT's and I'm hoping to get over 2000 in October. I took my subject tests in literature and us history; I got around 650 in both.

If I really work next year, will I at least be able to get into a nursing program in a second-tier college? I don't want to go to a community college or anything that holds the rank of a community college. I'm not aiming for ivy league either. So overall, will I be able to get into a college that's less than ivy league, but a lot better than community?

Some colleges I'm looking at right now are SUNY stonybrook and Rutgers New Brunswick.
