<p>If a school lists it as a required form, and you do not submit it, they will write you and ask for it. I assume you do not want to submit it because of something on it that will not help you? You are out of luck there.</p>
<p>More often than not, it is mandatory. </p>
<p>Instead of waiting to see if the school will write you asking you to send it in, you should just submit it from the get-go.</p>
