Question concerning Mid-Term Report

<p>I am applying to several schools for transfer admission.
Each school requires the mid-term report, which involves collecting a brief evaluation of the student from each professor.
Now, my questions:
Am I supposed to bring the form to each professor and ask him or her to fill in one section? If so, how would I do so and have each professor's trust in not looking at what they have written? After all, if I waive my right to access and therefore seeing the note, I cannot bring it to each professor.
Also, after having the form completed, would I simply send a photocopy to each school? Or, should I bring one form to each professor for every school I am applying to? This seems a bit excessive, and I do not want to waste my professors' time.</p>

<p>You have all profs mark/sign on the same page. While this looks like a contradiction of the instructions, the thing with FERPA is that you are signing away your rights to see the form after it is submitted, not before. So, for instance, sometimes LOR writers show the letter to the applicant before submitting and that’s OK, but the applicant can’t request to see the letter from the school if they sign. </p>

<p>My guess is that most profs will just write down your grade, and if they put any comments, they will only be positive and won’t mind you seeing them.</p>

<p>This is a good question. I think that copies would be fine, but to be sure call/email schools and ask.</p>
