School Forms ID #

<p>I gave my prof an instructor evaluation form I downloaded directly from the Common App site rather than the linked one under "school forms".. I filled out all my information, but I just realized that the copy I gave him doesn't have my common app ID number on the bottom of the page..! Should I write it on with ink or just leave it?...</p>

<p>I did the same thing. I didn’t know having your common application number on the bottom of the page was needed. Does anyone know the answer to this question? Thank you.</p>

<p>Yes, and birth date and all the other id stuff, just not the common app id… I mean, I don’t think it SHOULD be a serious issue, but still…</p>

<p>Well as long as your social security number is on the form then they will be able to match the instructor evaluation with your application.</p>

<p>I don’t think that ID# matters at all. Schools put up links to the generic forms left and right. If the number was necessary, or even just preferred, it would be set up so the forms had to be printed with the number all the time.</p>
