On my card it just shows the family account number and medical record no.
I've called CCN and they don't know either. Like *** lol.</p>

<p>yea its alrite, trust me. just provide what it asks you, it doesnt have to be right and you should be fine. They say they check but i think its an empty threat. ucsd personnel as lazy as hell.</p>

<p>i found out how to fill it and did it appropriately and i got an email from ucsd saying i was approved and the the ship was waived</p>
