Supplemental Recommendation Letter HELP

<p>What forms do I need to give someone who will write me a supplemental rec? I'm getting 2 teacher recs + a supplement one by my conductor outside of school. </p>

<p>I know I need to give him stamped envelopes with addresses, but what forms do I need?</p>

<p>You don’t need any forms, they just send the LOR. Oh yes, be sure they put your full name as it appears on your application, and DOB or identification number if the college has given you one (not your SSN).</p>

<p>If you’ve already submitted your application and they’ve given you some sort of number, have them use that, if not, have them put your full name and date of birth on the top or bottom of the letter so that schools can easily link it to your application.</p>

<p>crazy: think about it. They are in the business of accepting and processing 1000s of documents. It’ll get in your file. Don’t fret.</p>

<p>My piano teacher wrote me a rec, all he included was my name, high school and city and I didn’t even submit my supplement yet but the Duke admissions office confirmed that it wouldn’t be thrown out but establish a new file.</p>
