<p>If someone is writing you a supplemental recommendation (not teacher)… what type of information should they list and where on the recommendation so that it gets filed in an appropriate and timely manner? I don’t want them to lose the person’s supplemental rec, but I don’t know what information to tell them to put on it so that it gets filed (besides my name).</p>
<p>Student applicant number or ss#. This should appear on all pages of your applications papers.</p>
<p>what i did is i took the teacher recommendation form and edited it in photoshop and cut off the whole bottom of it so it's just applicant information at the top....</p>

<p>Yeah that should be fine. Hazmat, my supplemental letters of rec are already sent and they only have my full name, address, and birthday on them. My mom advised that I don't give out my social security number, but the information i provided should be enough right? I'm sure UPenn can figure it out...</p>
