Send it, they'll read it, but they'll still want 2 teacher recs (or 3 for science).

Just be careful that the recommendation comes from someone who really knows you and can provide insight into you that won't come from the teacher and guidance counselor recommendations. They really dislike getting recs from politicians or business leaders or alumni who don't know the student at all. Those could work against you.

What if the extra rec is from a politician who DOES know you very well? Is it the fact that the person is a politican, or the fact that they don't know you well that the adcoms don't like?

I sent two supplemental recs, from my workplaces.

Of course they don't have a sheet for it, how could you make a standard model? Just write the rec in whatever way you see fit. I'm pretty sure no one cares

d_rolla: They don't like getting recommendation letters from people who don't know you well. If the politician really knows you -- not your family, but you -- then sure, it's OK. If it's just a short paragraph that gives no additional information, I would skip it.

How about a recommendation from my internship mentor?

I sent in additional reccomendations and I strongly believe they had a very positive impact. However, they were very, very good, and said things the other recs didnt' say (thats' important, you don't want to be repetitive). You can either print out the form again or have them write a free-form essay.
