In the Application, they require an evaluation from a lab science instructor (physics or chemistry). Would it be possible for me to get an evaluation from my AP Biology teacher instead?

Check with admissions, but when my son asked this question (both his physics and chemistry teachers had left the school) he was told an AP Biology teacher could not be substituted.

I guess it depends, my AP Biology teacher wrote me a recommendation

so how did he get his last recommendation?

My son managed to track down his AP Chem teacher (school had a forwarding adress) - he agreed to fill out the recommendation. Son mailed the assessment form to him together with the WP return envelope and it was mailed directly to admissions.

does it matter what year english/math teacher is? I don't really want to give it to my 12th grade senior teachers as they've only known me for like 3 weeks, I would much prefer my junior math teacher and perhaps even my sophomore english teacher she knows me the best out of them all, and theres nothing wrong with my junior english teacher he just doesn't know me that well.

would the academy take this into consideration at all or will they be skeptical of somethin when they see she is my english teacher from 2 years ago? any insight would be greatly appreciated

tn48th - Yes, I agree that it would be better to use teachers from your junior year rather than a teacher who has only known you for 3 weeks in your senior year. I'm not so sure about using teachers from lower grades if you have taken a higher level of the subject. My son took AP Chem in sophomore year so that was the teacher he used for his chem recommendation, but it may look odd if you ask a sophomore English teacher to write your assessment rather than your junior year teacher. Others may may have more direct knowledge about this than I do, hopefully someone else will share.

Goos luck!

who are the three teachers(subjects) that I have to get my recommendation from? does it say on the application?

Should I just have my teacher fill out the evalution, or should I also ask them to write a formal letter of recommendation to accompany their evalution? If not, does anyone think it would be a good idea to send in a few letters of recommendation in addition to the rest of the application?

I would follow the instructions unless there is a reason not to. If something on the evaluation needs explaining a letter may be a good idea in addition to the evaluation.

If you look on each of the three "School Official" forms - they are not in front of me at the moment - at the top there are three check off boxes. One for English teacher, one for math teacher and one for Physics/chem teacher.
they should be from 11th or 12th grade

oh.. alright.. thanks queenbee89

They do not have to be from 11th/12th grade. However, they are specific about getting recom. from one english, one math, and one physics or chem. Last year when I went through the process my chem teacher had moved and I had just begun physics so instead of science I had my History teacher write me a recom. West Point sent back a letter saying my file was incomplete until I got a recom. in from a physics/chem teacher.
