i know most schools require two recommendation from secondary school teachers and they require two evaluation forms. if i am getting supplementary recs from my professors at community college do i need to give them those commonapp teacher evaluation forms or can they just write a letter?

I had my swim coach write me a supplemental rec, but he just wrote a letter for me. No forms, just a plain ol' letter.

yeah I'm having employer write a letter of reccomdation for me and its a plain letter, no attachment, nothing. A letter to the school you are supplying should suffice. However I would contact the college consulor about it just to make sure.

It depends on the school too...check the policy on their website; many give instructions for optional recs. I know Princeton, for example, provides a form for an optional supplementary recommendation. If it's not specified, contact the college if you're worried, but it'll probably be just an extra letter sent with your other forms.
