<p>I received an email today from the USC Cashier saying that they did not have a Title IV Authorization Form on file for me and as a result, my Title IV financial aid funds will not be applied to all charges. As I understand it, Title IV funds includes the Pell Grant, Federal SEOG grant, and subsidized/unsubsidized Stafford loans. My family will not be taking the loans (we'll simply be adding it to our EFC), but we, of course, would still like the grants. Did anyone else receive this email? Is it necessary to fill out the Title IV form to receive the grants? I am a continuing student, and last year, I did not have to fill out this form to receive my grants. Thanks, in advance, for your help!</p>
<p>I got the email too and I’m an incoming freshmen. I just got off the phone with fao and they said to just fax in the form and to do that each following year and it should not affect the student account, which should already be in order. Apparently, a lot of people received the email.</p>

<p>Another question, are you guys sending in both forms? Like one is “Authorization to Apply Title IV Funds to All Charges” and the other is “Authorization to Hold Excess Title IV Funds”.</p>

<p>Also, apparently it currently is not even required, and this whole form is only for future usage. At least, that is what the student financial services lady said. I’m currently just planning to send in the authorization form, so they’ll have it on file. I still don’t fully understand everything because the staff was not particularly clear in explaining both these forms. I think I’m just going to have the funds be applied because I’m taking my subsidized stafford and the workstudy. To confirm for yourselves, you can call student financial services 213-740-7471.</p>

<p>Basically what micL said, the forms aren’t required. They just sent out that email because it might become mandatory in the future so if you have the forms on file already, I guess it’ll save you the hassle later.</p>
<p>As for the two forms, the one about applying to all charges basically means it’ll apply to all the excess charges as well, like the extra money that’s charged to your account that’s not tuition and such.</p>
<p>The holding excess funds is basically… the Title IV funds don’t get applied to those excess funds.</p>

<p>When I called student financial services, they told me that they’ve been having problems or it was too busy. They told me that sending it over the weekend would be fine.</p>
