Do you offically have to let each college that you have been accepted to that you are denying thier admissions? If so, which is the best way to do this if there is no online form? Can we just email them?

If you are waitlisted at a school, do you have to tell them whether or not you decline their waitlist?

Or will it be assume for both that if you do not reply back by May 1st that you will not be attending that university?

Are there any consequences for not informing a school of your decision?

I don't think you officially have to inform a school that you won't be going and there probably aren't any consequences, but informing schools is just considered polite especially since they accepted you and all. Most school give you pre-paid postal envelopes and forms to let them know of your decision. I'm not really sure how to let schools know if they don't give you a form (maybe they don't really care and they'll just assume you're not going if you don't reply by May 1).

"Is it absolutely necessary for a college to tell me that I didn't get in, or is it OK if they just wait until April 1 and assume I'll just figure it out?"

If the answer to that question is No, then so is the answer to your question.

You need to let colleges know, for a number of reasons:
1) It's only fair to the kids on the wait list.
2) You never know where you might want to transfer to, or go to grad school. Burning bridges is really not a smart idea.
