UC Davis Supplemental Review Questionnaire

"Thank you for your interest in UC Davis for fall 2015. Undergraduate Admissions is carefully reviewing your application. It would help us to receive additional information from you in order to make the best possible holistic review assessment of your application.

Be sure to include your fall 2014 grades and spring 2015 coursework-in-progress. It is important to submit the questionnaire with the requested information within ten calendar days of this email.

In addition, you may also request support of your application from another individual (e.g., counselor, teacher, employer, organizational leader, religious official) who may complete and submit a Recommendation Form. To identify the recommending candidate of your choosing, provide their contact information in the area located at the end of your questionnaire. A notice to submit an online Recommendation Form will be sent directly by Undergraduate Admissions to the person you identified.

The supplemental questions ask about my leadership qualities and positions I have held.
According to this email, people told me that I was a borderline student. I’m not trying to sound haughty or anything, but I considered Davis to be my safety school and I believed that I could get in with no problem given my academic profile:

2300 SAT, 800x4 SAT2’s, 34 ACT, 4.4 GPA, and excellent extracurriculars.

Does this really mean that I am borderline? And more importantly, does this mean that I have a slim chance of getting accepted to Berkeley or LA?

A supplemental questionaire doesn’t always mean borderline. They are probably trying to get more clarification on your leadership EC’s listed on your application. With your scores and GPA, you may be considered for a Regents scholarship and they are just trying to get some additional information. Submit what they are asking for and Good Luck.

If you dont get one is that a sign?

Only a few applicants will get a supplemental questionnaire. It is not an indication of any acceptance or denial and vice versa. Admissions just needs more clarification.

you could’ve gotten the talent or disability one, not the borderline one. i hear theres three different kinds. so don’t worry, they don’t all mean borderline! like gumbymom said, they’re often used to scout for regents scholars

I received the 6-question “talent” questionnaire. These are the questions they asked:

  1. Please expand your description of the leadership role(s) you included among your list of extracurricular activities or mentioned in your personal statement on the UC application (school, clubs, sports, community, church, work). Please include the length of time you held the lead role and the level of responsibility (decision making) required.
  2. Please tell us what you learned (about yourself and others) as a result of your leadership role?
  3. Please describe more fully the special talent you included among your list of extracurricular activities or mentioned in your personal statement on the UC application (art, music, dance, sciences, athletics, etc.).
  4. Please help us evaluate your skills by providing us with information about the level of your achievement (school, community, regional, state-wide, national or international). Describe any awards/honors received, positions attained or prizes won in competition as a result of your talent (e.g., concertmistress of community-wide orchestra, participation in Junior Olympics for a particular sport, or placing in a national debate competition).
  5. What impact, if any, has your pursuit of this/these endeavors(s) affected your academic achievement?
  6. How will you further develop and use your extracurricular experiences if you are admitted to UC Davis?

So do you mean that the questionnaires vary depending on your status?

I got a supplemental questionnaire as well, but mine was 18 questions which incorporated all 3 “types” of questionnaires I think, what does that mean? Also, I got a 30 ACT and a 4.33 gpa as of now.

From my limited knowledge, I believe that the 18 questions signify a “borderline” applicant. Don’t count on it though. I could be wrong.

But that’s quite odd. Your GPA is above the average and your ACT score is just about average. Theoretically, you should have no problem getting in, but who knows?

I agree with the post above. I have been searching CC for information on UC Questionnaires and it seems like there are the 3 types of questionnaires you mentioned: Special talent, disability, and Borderline. The 18 question questionnaire seems to be intended for borderline applicants. I think you have a good chance of getting in if you write thoughtful responses, as your stats are above the norm for Davis. Good luck.
