Does anyone know if taking the titer tests to prove immunity is required for the UF imminization form. They have a section for it on the immunization form; however, on the information/instruction page it does not say if it is required to be filled out or not.

You only need titers done if you can’t get access to records of when you received any of the required vaccinations. For example, I had to get titers drawn because I’ve moved around a whole bunch and trying to track down the doctor’s office record for each of the vaccinations would be impossible. But if you had a regular physician growing up (or parents who keep track of your records), then you should have record of when you received all your vaccinations.

also, the health departments have records of all the shots that you have gotten if you got them in that departments county/state. I know that I had absolutely no proof of my shots, but I just went to the health department in my county, gave them my ss#, and they gave me a sheet of exactly when and where I had received all my shots.

I got my records from my high school. I went to the Health Department first and they didn’t have my info.

The reason I asked about titer dates is because UF keeps rejecting my immunization forms. I’ve had them filled out by the dr. 3 times, and the only thing not filled out on the form is the titer dates. All of my immunizations are current but for some reason they are rejecting the form.

Call them and find out why… there has to be another reason. it says directly on the immunization page “Laboratory evidence in the form of blood titers for Measles and Rubella immunity is also accepted in lieu of proof of immunizations.” So the titer thing def isn’t it… maybe something else is missing or incorrectly filled out.

I remember that before going to UF, the immunization form was the first one I did, if necessary take the vaccines again or any test that is required, and it will save you some time from the hold in ISIS and for registration in Preview.
