<p>So I want to waive out of SHIP since I have free health insurance at least one year. I am not sure if I am eligible since the requirments to waive SHIP is </p>

2.Have at least $100,000 in lifetime benefits. </p>
<p>What exactly is those above requirements? Can I assume that I qualify? I don't even know how to figure out if I am eligible.</p>
<p>These are things you have to ask your current insurance. Usually if they have a brochure about your coverage, it includes information like this.</p>

How do I know if my insurance meets the waiver criteria?</p>
<p>If your insurance company is a US owned company that is headquartered and operating in the United States and your plan is not a travel insurance policy or a reimbursement program, we recommend that you contact your insurance company and ask the following questions:</p>
<p>a. What are my lifetime benefits under my current plan?</p>
<p>b. What is my maximum per year out-of-pocket expenses for hospitalization, surgery and emergency care? Specifically, you may want to ask what your annual deductible is and what your maximum out-of-pocket limit for co-pays and co-insurance expenses is. The sum total of your annual deductible and your maximum per year out-of out-of-pocket limit for co-pays and co-insurance expenses equals your maximum per year out-of-pocket expenses for hospitalization, surgery and emergency care.</p>
<p>c. What is the closest facility that is covered under my plan to UCLA? </p>
<p>If your lifetime benefits is $100,000 or more AND your maximum per year out-of-pocket expenses for hospitalization, surgery and emergency care is less than $5,000 AND the closest facility covered under your plan is under 25 miles from UCLA, then your insurance meets the waiver criteria. </p>

<p>I am going to waive the SHIP too, and the information I’ve seen has directed me to a page on the Ashe Center site that lists the fees for different services (<a href=“http://www.studenthealth.ucla.edu/_doc/AsheFeesFlyer.pdf[/url] ”>http://www.studenthealth.ucla.edu/_doc/AsheFeesFlyer.pdf</a> ). As far as I can tell, most things cost $12. Am I reading that right? (i.e., the Fee for Service doesn’t also apply, right?)</p>
<p>what i did was call my 1-800 number for my insurance and directly read them the questions, and omg it was awesome.</p>
