What do I do with the College Official Report?

Hi, I am a freshman student at the New School (Eugene Lang) and I absolutely hate it, so I am applying to transfer for next semester. One of the colleges I've applied to requires 2 teacher recommendations and a college official report. I have one recommendation sent in, but I still need to complete the 2nd, and the college official report!

What is the college official report and how do I take care of it, in other words who should I take it to, and if I need to take it to my dean how do I take it to my dean, basically how do I do anything relating to the completion of the college official report.
Any help is much appreciated, thank you!

The purpose of the COF is basically to determine if you are in good standing with your school. There is an evaluation on the form, but it means little as colleges know that whoever fills out this form is unlikely to know you well, if at all.

Yes, take it to your Department or the Registrar of your college and ask them who fills it out as it varies by school, it could be your advisor, Dean, Registrar, etc.
