I sent mine right before they should have been in. so it hadn't been too long, and it could also work as a reminder in case they forgot.
I'm going to follow up with an email or something once I get in.

You should thank them before you find out whether or not you get in. That way, you'll show that you're thankful no matter the outcome since they took time out to write one.

I gave thank you cards to my teachers about a week after they gave me the finished recommendations.

I'm no expert but I gave my teachers both just a nice box of Godiva chocolate with a little thank you note, after they'd written the recommendation but before I'd been accepted anywhere.

I think that way it shows that the thank you is for them taking their time and helping you, regardless of whether you get in.

^good idea.
I would've done that, my I had a director from a summer workshop I've done for years write mine and I gave her chocolate as an end of the program thing. so doing it again would've been a bit much.

Give the notes shortly after they write them. Your giving a "thank-you" shouldn't seem to depend on whether you get into the colleges.

When you get into the colleges, let the people who recommended you know -- either in writing or tell them in person, adding another gracious "thank you."
