My mentor in the lab I worked last year wrote me a recommendation letter, but I do not know where to submit it.
If I submit it as an Additional information form in the Writing section of Common App, should I submit it as a PDF so that I can include my mentor's signature? If I do it that way, I will have to see the letter first. But I am not supposed to see the teacher's recommendation, is the Employer letter the same way?
If I cannot do it the way above, should I require my mentor to send the letter directly to the admission offices of the schools I am applying for?
Thank you!

You may give your mentor addressed and pre-stamped envelopes for each school sent to their admissions address and have them mail the rec. Have them write somewhere on the letter your common app ID to make it easier for the school to match it with your file.

Thank you so much and I really appreciate your help.
One more question: What can I submit as Addtional Information so?

Additional information is exactly that. Info that is not repeated from your EC list. Some people have had numerous awards, performances etc in music that they can use that section for. You could elaborate on something/things that could not fit in the EC section but something worth telling.

Other people use it to explain inconsistencies in their App to explain things.

Do not write another essay, do not make it more than about a page, do not include an entire research paper of multiple pages etc.

I know its presence makes one feel they have to fill it. But the adcoms are only going to glance at it so make it concise whatever it is.
