About the Common App school forms

When do I give the Midyear and Final reports to my counselor? Do I hand them in with the Secondary School Report? If not, when?

2.Also, what do I hand in with the Secondary School Report? Just an envelope to each of the colleges I’m applying to, right?

Is my counselor supposed to send my transcript with the Secondary School Report, and in the same envelope?

What do I have to give to my teachers with the teacher evaluation? Again, just an envelope to each of the colleges I’m applying to, right?

Is the first page of the teacher evaluation supposed to have a big blank space at the end?

What’s the difference between Teacher Evaluation 1 and Teacher Evaluation 2? They just look the same to me, so does it matter if I give #1 to both teachers, #2 to both teachers, or #1 to 1 and #2 to another?

There’s four marking periods at my school. I’m taking Criminal Justice for the first two of them (the first half of the school year), and World Issues for the last two. Which class do I write under “First Semester/Trimester”, “Second Semester/Trimester”, and “Third Trimester”?

And lastly, stupid question, but, does it matter if I print out these forms in black or in color?

Also, on the Academic Honors section of the Common App, how do I have to list the awards? Is it like:

award - description/grade/whatever

award - description/grade/whatever

award - description/grade/whatever

award - description/grade/whatever

or like:

award - description/grade/whatever; award - description/grade/whatever; award - description/grade/whatever; award - description/grade/whatever

For questions 1, 2 and 3, ask your guidance counselor. He/she's done it before and knows your high school's procedures.

For question 4, ask your guidance counselor. Some kids give an acitivity list, some kids give a list of the schools with the due dates for the recommendations, some kids just give the form and the envelopes.

Question 6: Doesn't matter.

Question 7: Two quarters make up a semester. So first semester, put Criminal Justice; for second semester, put World Issues. Leave Third Trimester blank.

So I leave all spaces blank under "Third Trimester"?

Yes. All blanks under Third Trimester - that's only for schools that have trimesters.

One of my colleges requires "one letter of recommendation from the college counselor". Does the Secondary School Report count as this, or does an actual letter have to be attached to it?

Once again, Strawberry - ask your guidance counselor. He or she is the one who will do the forms and/or letter. Let him/her do his/her job!

Can anyone answer these questions?

"5. Is the first page of the teacher evaluation supposed to have a big blank space at the end?"

"8. And lastly, stupid question, but, does it matter if I print out these forms in black or in color?"

Doesn't matter. Your teachers will photocopy them anyway, and unless they have a color copier, they'll be in black and white.

Awards - doesn't matter, as long as it's readable.

Since I haven't printed out the recommendation forms this year, I don't know, but my guess is that yes, they're supposed to have a blank. Even if they're not, if all the info appears, again, it doesn't matter.
