<p>To answer this and the other thread ("urgent question"), yes, you do have to submit other forms.</p>

<p>If you scroll down to the part II of the application (step 2), you will see a list of instructions. Among them are other forms to download. I believe you must send in forms 3 (academic history), 4 (letter of recommendation), and 5 (test scores - yes, you are advised to send them in even if you sent in the official score reports already).</p>

<p>Oh dear... if you havent sent in your letters of rec... Hopefully you've already given your teachers/counselors the forms to at least start writing, or it'll be hard for them to get the Dec. 1 deadline.</p>
<p>I believe letters of recommendation can come later than Dec 1. Only your application must be in by Dec 1.</p>
<p>But it would be a courtesy to them to get them in ASAP.</p>
