<p>For the USC application, there are three supplementary forms to be downloaded: Freshmen SAT/ACT Student Report, Freshmen Teacher or Counselor Report Form, and Transcript Request Form for High School Records. I have a few questions regarding the forms.</p>
<p>1) For the SAT/ACT Student Report Form, do I really have to print that out and photocopy the score report I got in the mail from collegeboard? The reason I ask is because I already had my scores sent to USC in the first place, so I don't know what exactly I need to send them.</p>
<p>2) For the Transcript Request Form for High School Records, I obviously do not have my senior grades in at all and my current schedule doesn't appear on the transcript, it only shows my grades/classes from 9th grade to 11th grade. So, do I just submit that since I obviously can't provide them with graduation verification since it'll be quite a while 'til I get my senior year grades and the due date is December 10 for scholarship consideration.</p>

<p>2)umm... just go to your counselor and have them send your transcript. 99% of freshman applicants are still in high school, so i think the USC adcoms will be able to figure it out ;-)
There is another form that is a student-reported academic history, which you can use to write in your senior year schedule. you write "IP" for in progress in the slot for "grades."</p>
<p>going back to reading my own thread, I think for my first question I sounded sorta rude when I didn't intend to. When I said "I don't know what exactly I need to send them," I was referring to whether I needed to send them the score report I got in the mail or the one on my "My SAT" page.</p>
<p>(Don't worry about your tone- we all know that the application process is very stressful. There have been a lot of repetitive questions here and I think it sometimes gets frustrating for the regulars.)</p>

<li><p>You don't have to self-report your scores if you had them sent and you know they will get them by the deadline. If you want to be absolutely sure they have them, yes, photocopy ANY of your reports and send it in.</p></li>
<li><p>Yes, socks is right. Just have your available transcripts sent. You will be asked for mid-term grades when they are available.</p></li>

<p>Filling out the transcript request form depends on your high school. At my kids' school, transcript requests were done online so we did not turn in that form. If your school does it manually, then yes, you do fill that out to give to your records department.</p>
<p>? You either order the transcripts online or you hand the form to your records/transcripts department at your school. The form is very basic. You don't have to have anyone to do it for you.</p>
<p>Maybe you mean you don't have a records department? Then ask your counselor. There has to be someone at your school that sends transcripts.</p>
<p>The Trascript Request form says "Part I - Student" and never says counselor or teacher or anything. It's form seven. I'm guessing I fill that one out myself. But no, I don't have anyone to send this stuff for me. I have to do it myself. My counselor is also...lacking in college knowledge. =/</p>
