<p>Okay so I'm filling out the application using the CommonApp....do I have to send them the "Teacher Evaluation" form from the Common App website AND the "Counselor/Advisor Evaluation Form" from the Univ. of Miami website by MAIL even tho the common app is online...???</p>
<p>I would assume you just have to send the Miami one. Pretty sure of it. But you might want to contact them.</p>
<p>if your teacher is writing you a specific recommendation with an official school letter head, do you still have to have them fill out any form? & is this teacher recc. form for everyone to have filled out, or just for certain schools/programs?</p>
