Filling in the College Report form

<p>From what I can gather by looking at it, it seems as if it should be filled in by your College Adviser (a.k.a. Academic Adviser) - I don't think the professor I'm using to fill in my "Instructor Evaluation" has access to my academic record and disciplinary record.</p>

<p>However, this means I have a problem when it comes to filling in the "Ratings" section and the "Evaluation" section. I have a new academic adviser now (my old one left just 6 weeks ago), and he obviously doesn't know me well enough (I've not met him once, yet) to put almost anything - let alone anything meaningful - on these parts of the form.</p>

<p>What should I do? Leave those sections empty? Will admissions people throw my application out then?</p>

<p>The evaluation section of the College Official’s Report form is not important for the reasons you stated. Have your advisor indicate the short length of time they have known you and that they are not in a position to give a full evaluation. It won’t hurt your application.</p>

<p>You first need to find out who should be filling out that form at your school. They have done it before and can answer your questions. At my child’s school, the from is NOT filled out by an advisor; rather it is filled out by the Dean of Students at the college.</p>
