In Part II online access, I am able to see form 1 - form 5. Now I get form 7 from your link.
But then it must have form 6 some where, right?</p>

<p>For your midyear report you will enter your grades to an on line link on your application page and mail in a photocopy of your report card. No form will be required.</p>

<p>How many recommendations are needed?</p>
<p>The website says:
•Form 4, Counselor/Teacher Report, with attached letter of recommendation (may be sent separately).</p>
<p>SO, a form with a letter. Why does it say “may be sent separately”? Does it a counselor write the form, a teacher write a letter???</p>
<ul>
<li><p>USC Requires one letter of recommendation, and it can be from a counselor OR a teacher.</p></li>
<li><p>USC will accept and consider more than one recommendation, and many students submit two or three.</p></li>
<li><p>If you decide to submit ONE recommendation, both the form and the letter should be completed by the same person. The “may be sent separately” means the teacher or counselor can send the recommendation directly to USC - it does not have to be included with other materials you are submitting.</p></li>
<li><p>If the teacher or counselor hands the recommendation to you, it is fine to put it in a larger envelope with your other materials. The recommendation should be in a sealed envelope and is typically signed across the seal by the recommender (though that is not required).</p></li>

<p>No, I don’t have time! In a public a school, my GC is working with 750+ students, so the school usually requires seniors to make requests no later than 10/1 or 10/15. After these dates, no more schools can be added!</p>
<p>By the way, I <em>heard</em> USC requires a 9 x12 envelope with three ‘forever’ stamps to put recommendation letter. Is that so? Not the business letter envelope?</p>
<p>You may send your materials in any envelope you choose and with the postage of your choice. USC is very flexible about how you submit things. Whomever you <em>heard</em> that from is not a reliable source.</p>
