At the end of my sophomore year, various clubs and boards had officer elections and I got positions in 5 of them for the upcoming junior year. However, I transferred to another school during the summer and could not actually hold up to those positions. After a year at my new school, I did end up getting 3 leadership positions for my senior year. I plan to apply to top colleges, and I’m wondering what I should do about the nominal leadership positions that I was elected for but didn’t actually follow up on.

I would include the fact that you were elected and then include a note that you were unable to serve due to changing schools.

In Michele Hernandez's "Acing the College Application", she recommends against using a resume and instead strongly recommends that you use an activity list that you prepare in Microsoft Word. The activity list is an appendum that is in the same format as the activity box on the application form. This box has the titles: Activity, Years, Time per week, Weeks per year, and Positions of Responsibility. The appendum is attached since the box on the form is too small for all the info.
