<p>My teacher wrote a rec without using the common app form. What other information do I need to include on the paper before faxing it in? Date of birth? Where can I see my common app id number? Can I just write these things on the paper?</p>

<p>I would write your full name, date of birth, and SSN. They should be able to find where it needs to go. Just make sure to include your SSN on your Common App.</p>
<p>Bionic, UVa may not require a rec (I actually was under the impression that they required 1) but it is beneficial to send one in.</p>
